Our office does not participate with any insurance program and we request all payment be made at the time of consultation. However, this does not rule out the possibility of reimbursement by your insurance provider. We will be glad to provide you with properly coded invoices or HCFA claim forms that you may submit to your insurance company for reimbursement. If you wish, we can forward the claim for you. Some insurance plans cover the cost of office visits according to their out-of-network provider schedule. Please contact your insurance provider for more details regarding coverage.
Our offices accept either cash, check or credit card and some flex spending cards(depends on how your company has your card set). If your insurance company requires that we submit a report or notes we will furnish that as well. We do all that we can do to help you with your reimbursement.
